It is the patient's responsibility to contact their insurance company prior to their visits to ensure insurance coverage, verify benefits, co-payments and deductible amounts for the services received in our office.  If necessary, we offer convenient monthly payment plans with CareCredit.  For more information, contact them at www.carecredit.com or 1-866-893-7864.

 *Laboratory Billing & Insurance:  Our office currently uses Rex Healthcare/Rex Outreach for the majority of our laboratory testing needs.  Rex employees are located inside our office providing laboratory services.  Rex bills separately for their services and it is the patient's responsibility to ensure their insurance company participates with Rex.
