A $20.00 fee will be charged as patient responsibility for the completion of forms and/or letters and must be paid prior to the release of the forms, including but not limited to:

*disability forms for maternity or surgery*FMLA forms*Leave of Absence forms/letters*forms/letters to employers

A minimum of 5 business days should be allowed for completion of all forms/letters.
